The Concordia Communities will be designed to provide different kinds of housing and multiple levels of personal care. To meet the individualized needs of all of Concordia’s residents, each of the different Residences will be separately situated, but architecturally combined into one campus-like complex. Each of the separately situated Concordia Facilities will have its own dining rooms, lounges, parlors, activity rooms, pantry kitchens, a 24-hour emergency response system, and, of course, its residential units. Concordia’s Commons Area will be strategically located so as to allow the residents in all of the Concordia Facilities to access specialized services, participate in senior-focused programs and to connect with each other and the public.
Depending on the size of the Concordia Community, the Independent Living Residence will be either detached dwellings or clustered townhouse units or apartments suitable for single occupancy or for couples and designed for active seniors.
Each Independent Living Residence will feature spacious floor plans, a full kitchen, a washer and dryer and handicap accessible bathrooms.
The Independent Living Residences will be offered to those seniors who wish to enjoy an active, independent and carefree lifestyle without the burdens or worries of home ownership. Instead of facing the unpredictable costs of home ownership Concordia’s residents will occupy their Independent Living Residences at a fixed rental. Residents will also enjoy the freedom to join in a diverse array of social, educational and cultural activities as part of Concordia’s Coordinated Activities Program and will also have available to them services such as transportation, fine dining with delicious restaurant-style meal plans, activities, wellness programs and weekly full housekeeping services.
What is the difference between the Enriched Housing Program, Enhanced Assisted Living Residence and Special Needs Assisted Living Residence?
The Enriched Housing Program will be designed for seniors who are in need of only a moderate amount of assistance with their daily living activities, such as grooming, dressing, bathing and toileting, as needed. The Enriched Housing Program is a facility that requires licensing from the New York State Department of Health. Concordia’s Enriched Housing Program will be licensed and will be operated in accordance with the regulations promulgated by New York State’s Department of Health.
Concordia’s Enriched Housing will be a separate village of private apartments, suitable for singles or couples. Each resident will be entitled to personalized care based on the needs of each of them, including 24-hour supervision.
Like the Independent Living Residences, the residents of the Enriched Housing Program will receive full housekeeping service, laundry services, three balanced meals daily, transportation and participation in a wide array of diverse social, educational and cultural activities as part of a coordinated activities program. In the Enriched Housing Program, Concordia’s Signature Program will offer expanded services that will include access to physicians for medical care, assistance in managing disabilities and regular consultations on ways to maintain a healthy lifestyle.
The Enhanced Assisted Living Residence will be designed to care for those seniors who suffer from chronic illnesses or disabilities that require advanced levels of care and skilled nursing services. As is the case with the Enriched Housing Program, the Enhanced Assisted Living Residence must be a Licensed Adult Care Facility and certified as an Assisted Living Residence. Concordia’s Enhanced Assisted Living Residence will be operated in accordance with regulations promulgated by the New York State Department of Health. In the Enhanced Assisted Living Residence, Concordia, in consultation with physicians, will devise an individualized plan of care and supervision of each of its residents in this Section. As in the case of the Enriched Housing Program, Concordia’s Wellness Program will provide the residents of the Enhanced Assisted Living Residence with comprehensive medical care from on-site physicians, frequent consultations with Concordia’s nursing staff to detect symptoms that need attention, assistance with medication management and regular consultations to help maintain a healthy lifestyle.
The residents of the Enhanced Assisted Living Residence will receive the same amenities as are provided in the Enriched Housing Program, such as three balanced daily meals, full housekeeping services, laundry services, transportation and participation in an activities program specifically designed for the needs of the residents.
The Special Needs Assisted Living Residence will be a private secure section specifically designed for the comfort and safety of residents suffering from cognitive impairments. The operation of the Special Needs Assisted Living Residence also needs to be licensed by the New York State Department of Health. Here also, the Special Needs Residence will be operated in accordance with the regulations promulgated by the New York State Department of Health.
In the Special Needs Residence, Concordia’s activities and wellness programs will be specifically designed to meet the needs of the residents with memory impairments. In addition, the residents of the Special Needs Assisted Living Residence will also receive three balanced meals daily, housekeeping services, laundry services and transportation when needed.
Whenever possible, Concordia Communities will offer both Adult Day Services and Respite Care.
Concordia’s Enriched Housing Program will operate the Adult Day Services Center for seniors who live at home, but are in need of supervision during the day when family caregivers are not available for that purpose. Transportation to and from the Concordia Community will be provided by Concordia. The Day Services Center will provide meals, recreational and social activities, occupational therapy and, at the option of the senior participant, membership in Concordia’s Signature Program.
Concordia will also reserve and keep available residential units that will give family caregivers the opportunity to place their loved ones with Concordia temporarily and on a short term basis to allow for caregiver vacations or merely as a respite from the duties and responsibilities of caregiving.
